ECU does not have enough power capacity thru the CAN to drive the heater in the O2 sensor. Get the power from the Injector power feed that is relayed. That way it won't backfeed.

Yes normally with ignition on there should be +12v on pin D of the coils.

From what you posted, I understand you do not have a Home (cam) sensor. Then group the coils like the injectors and select semi-sequential or batch for your 4 cylinders. During startup, the base fuel table or any Correction Tables are not used until the max cranking rpm is reached. Then it is considered in a "running" condition.
Only Prime Pulse and Cranking tables are used during startup.
